(a)Any individual, partnership, corporation, or other entity that provides dental services through telehealth shall make available the name, telephone number, practice address, and California state license number of any dentist who
will be involved in the provision of services to a patient prior to the rendering of services and when requested by a patient.
(b)A violation of this section shall constitute unprofessional conduct.
